- All participants must submit a waiver prior to joining a club sport. Click here to access the waiver.
- All club sports volunteer coaches/instructors are required to fill out coaching agreement form annually. Click here to download and complete the agreement.
- The driver’s agreement and injury report forms can be found on the NSU Club Sports SharkHub page.
- The Recreation and Wellness Department for Club Sports uses a tier classification system to allocate club sports funds among all active clubs. Learn more about this system.
- For information about funding club sports visit the Student Activity Fee Accounts Office.
